The study sample was derived from the Eplerenone PostAcute Myocardial Infarction Heart Failure Efficacy and Survival Study EPHESUS ; Quality of Life Substudy, which has been previously described.4, 5 In brief, EPHESUS was a multicenter international clinical trial that examined the efficacy of eplerenone, a selective aldosterone blocker, on reducing morbidity and mortality among patients with HF after an acute MI. Between December 27, 1999, and December 31, 2001, patients meeting the following inclusion criteria were enrolled: evidence of an acute MI as documented by standard criteria, left ventricular LV ; dysfunction defined as an ejection fraction of 40%, and HF as documented by physical examination findings or chest radiography showing pulmonary venous congestion. All patients were enrolled between days 3 and 14 after their MI and received cardiac treatment at the discretion of their physician s ; , which could have included ACE inhibitors, angiotensin-receptor blockers, -blockers, digoxin, diuretics, nitrates, antiplatelet agents, and coronary reperfusion. For the Quality of Life Substudy, the KCCQ was administered to all EPHESUS enrollees in the following countries: Argentina, Belgium, Brazil, Canada, France, Germany, the Netherlands, Spain, the United Kingdom, and the United States. INTRODUCTION Malaria, a term which stems directly from two Italian words mala and aria, meaning bad air has plagued mankind since ancient times. It is among the oldest recorded diseases. Chinese mythology described three malaria demons, one with a hammer, another with a pail of cold water, and a third with a stove. They were responsible for the characteristic headache, chills and fever. The classic attack of malaria is recognized by its periodicity and pattern. However, today the classic clinical picture is more of an exception than the rule particularly with falciparum infection, which explains why it can easily be mistaken for other diseases with catastrophic results. Malaria is probably the most widespread of diseases in the tropics and is now once again appearing in the developed countries of the west. The World Health Assembly in its 31st session reported a resurgence of malaria in many countries. In the Philippines likewise it remains as one of the leading causes of morbidity and mortality. It is the purpose of this paper to study the clinical features of malaria, to expose the pitfalls in diagnosis and to evaluate the effect of current antimalarial therapy. MATERIALS AND METHODS One hundred seventeen patients were hospitalized for malaria in the Santo Tomas University Hospital from May 1970 to May 1980. Fifty five cases showed positive blood smears. The remaining 62, although showing dramatic therapeutic response to anti-malarials were not confirmed parasitologically. For purposes of this study, only patients with positive blood smears were considered and their clinical records reviewed. RESULTS AND DISCUSSION The age and sex incidence of the 55 cases are shown in Table l. The youngest was two and one-half months old and the oldest was seventy years old. Majority belonged to the 2nd and. 3rd decades of life. Sixty nine percent were males and 31% were females. Although our study showed preponderance for the males, sex is not a direct factor in malaria but may influence rates through occupation, the type of clothing used or relative mobility and epogen.

Mountains, flowing first to the north-east and then east-south-east to join the Olifants River inside the Kruger National Park shortly before it enters Mozambique. The "perennial" of the Great and Middle Letaba rivers are only perennial in their upper reaches, becoming progressively more seasonal and episodic as they flow eastwards. The three episodic tributaries rise at different points in the drier eastern portion of this subcatchment within the Kruger National Park. From the mid-1960s, flows in the Great Letaba River have declined progressively sue to rapidly increasing water abstraction in the upper reaches. For the last twenty-five years, flows in the lower reaches of this river have been entirely seasonal with no surface flow recorded during the dry winter months. This has posed several problems for the Kruger National Park, as the Great Letaba River is an important source of water for game animals. Accordingly, several small reservoirs were constructed along the lower reaches of the Great Letaba River inside the Kruger National Park to provide assured supplies of water during the dry winter months. There are several water supply dams and reservoirs located in the headwater zones of all the tributaries of the Great and Middle Letaba rivers. These reservoirs supply water for domestic consumption in the several towns, as well as irrigation water for the extensive areas of irrigation agriculture at the foothills of the Drakensberg Mountains and along both banks of the larger rivers. In the drier central and eastern portions of this subcatchment, the many settlements, farmsteads and small towns rely on boreholes or run-of-river abstraction for their water supplies.
